Nursing Home Survey Readiness

The survey risk scan costs $6,500, and quarterly updates add $250 a month. Using CMS's public inspection, fine and staffing files, it compares your building with others in your state and ranks the tags (the federal requirements surveyors cite by number) where you are most at risk. Your director of nursing gets an audit checklist for each one. Your team chooses every fix.

Built for

  • Administrators and directors of nursing with a survey window opening
  • Homes with a recent fine or a falling inspection rating
  • Regional clinical leads comparing buildings against their states
  • Operators who want evidence kept current between mock surveys

Why it's worth it

Today

A mock survey or an internal audit shows where you stand on the day it runs. A stale policy or a lapsed training record can sit unnoticed until a surveyor cites it, and among homes fined in the past three years, the median total was $33,594.

After

Before the survey window opens, you know the tags where your building is most at risk, with an audit checklist for each. If you add the readiness checker, your evidence for those tags is checked every month and each gap goes to the person responsible.

Why it matters

44.3%

44.3% of US nursing homes were fined at least once in the last three years, and among those homes the median total in fines was $33,594.

How it works

A person on your team approves every result

The tool runs these steps on a schedule. Nothing moves past the approval step until someone on your team signs off.

  1. InputCMS files and your recordsCMS's public inspection, fine and staffing files for your building and state. For the checker, your policies, training records and audit logs in SharePoint or Google Drive.
  2. CodeRankCitations, fines and staffing compared with your state, tag by tag.
  3. AI modelDraftAudit checklists for the top tags and, after a survey, plan of correction drafts, all with an “AI draft” label.
  4. A person approvesDirector of nursing decidesThe director of nursing and medical director edit every checklist and choose every intervention.
  5. OutputReport and binderA ranked risk report in your building's name for your quality committee, editable checklists and, with the checker, gaps sent to each owner.

What you get

Risk

A tag risk list

Your citations, complaints, fines and staffing compared with homes in your state, ranked by how often and how severely each tag is cited. A tag is a federal requirement surveyors cite by number, such as F880 for infection control.

Audits

Checklists for your top tags

An audit checklist for each high-risk tag, drafted by AI from the regulation and your state's recent citations. Each is labeled as a draft and arrives as an editable document for your director of nursing.

Evidence

Kept current each month

With the checker, your policies, training records and audit logs are matched to each high-risk tag every month, and each gap goes to its owner by email or Teams.

After a survey

Plan of correction drafts

A first draft of each plan of correction, the written fix a home files after a survey, from the statement of deficiencies and its resident codes only. Your director of nursing and medical director choose every intervention.

What done means

The tag risk list and checklists are delivered, and your director of nursing has walked through them with us once.

With the readiness checker, the first monthly gap check has run without help, and each owner has gone through their open gaps once.

Choose your version

Each version is a one-time build, then a flat monthly fee. The fee covers running the public checks on our servers and looking after the part of the tool that runs in your own account.

What the monthly fee covers

On Scan and watch, the $250 a month pays for us to refresh your risk list each quarter from CMS's latest files, flag new citations in your state, and hold a quarterly call with your director of nursing. On the readiness checker, the fee pays for us to watch every monthly check, fix it when your document system or a CMS file changes, keep it working as AI models change, and check in each month. Either way we reply by the next business day, and it's month to month with 30 days' notice. The public scan runs on our servers, and its hosting and AI usage are included. The checker runs in your own account, so you pay its hosting and AI usage directly, and your proposal estimates that bill in writing before you sign. We set the accounts up with you during the build, and their bills come to you. If you stop, the checker keeps running, and you get every finding from the public scan as files.

Payment and terms
  • Under $20,000, half at signing and half at delivery. From $20,000, 40% at signing, 30% at a mid-build demo and 30% at acceptance.
  • Paying by purchase order? There's no deposit: we invoice half at kickoff and half at acceptance, or from $20,000, 25% at kickoff, 35% at a mid-build demo and 40% at acceptance. Acceptance means the “What done means” criteria are met.
  • Not legal or clinical advice. Your director of nursing and medical director choose every intervention, and the tool never marks a tag met.
  • The monthly fee is month to month with 30 days' notice, and we reply by the next business day. The part of the tool that runs in your own account is yours on final payment and keeps running if you stop. The public checks on our servers stop, and you get every finding and draft as files.
  • The fee for a smaller first step counts toward the full build if you buy it within 90 days.
  • Running three or more of our AI tools? Their monthly fees are capped at $1,000 a month in total. Health system, network and multi-site versions are priced on their own.

Year one is the build plus 12 months of the monthly fee. Hosting and AI usage for the part that runs in your own account are billed to you directly by those providers, and your proposal estimates them before you sign. The build times on this page are measured in weeks of full-time work. Start and finish dates are set in writing in your proposal.

Where this comes from

It follows the same design as Sentinel, a system Tyler built that checks a live data platform against its sources every morning and sends each finding to the team responsible. The TEAM hospital explorer on this site, built from public CMS data on the hospitals in Medicare's TEAM payment program, shows the same kind of work.

Questions

How common are fines?

In CMS's September 2026 file, 6,510 of 14,690 nursing homes, or 44.3%, had been fined at least once in the past three years. Among those homes, the median total in fines was $33,594.

Is there a free version?

Yes. Ask for a one-page snapshot of your building's citation and fine history against your state, built from the same public files.

Our state association already runs mock surveys.

Keep using them. The scan adds a ranked tag list from the public data, and the checker keeps the evidence current between mock surveys.

Does it look at resident records?

No. Anything that reads resident records, such as MDS or care plan audits, would need a BAA and isn't available yet. The plan of correction drafts use only the resident codes already in the statement of deficiencies.

Do you cover hospice or home health?

Not with this tool. Tracking the hospice cap needs each patient's days of care, so we can't offer it until a BAA is in place. For home health, the operator version of the Weekly Leadership Brief tracks your quality scores.
